This form helps us gather the key details we need to plan a smooth, enjoyable and well-organised photoshoot at your nursery or school.

By completing this form, you’ll help us:

• Understand your setting and schedule

• Prepare the right setup and equipment

• Support all children, including those with SEND needs

• Ensure we arrive at the right time and follow your safeguarding processes

• Deliver a calm, safe and efficient photography experience for staff, parents and children.

Please fill in as much information as possible.

Once submitted, we’ll confirm with a Service Agreement that also outlines how we operate on the day and include all the legal bits, Consent forms for you to print and distribute to parents/guardians and any promotional material advertising the shoot day.
